Complete this form to submit a complaint or compliment about a Metro Transit employee or transit service. Please be sure to be as complete as possible so that we can identify the employee or transit service (bus route or train) and forward your comments to the employee’s supervisor or department to take appropriate action.

Date and time pickers
Include the time you boarded the bus/train to assist us in locating the correct vehicle.
Include bus route number or METRO line. Example: 12, A Line or Blue Line.
Be as specific as possible such as Stop Number, station name, intersection or vehicle number.
Located near the door of the vehicle or by the driver.
Employee number is on the patch on the right shoulder of the operator.
Please provide as much detail as possible. If the time and location of an occurrence/incident is different than the time you boarded, please make sure we have that detail in this section.
File type can be gif, jpg, jpeg, png or pdf. The maximum file size for jpg/jpeg files is 10mb. The maximum file size for gif, png and pdf is 4mb.
